Description
I AM enables individuals with intellectual and developmental disabilities to reach their hopes and dreams by identifying person-centered activities, services and supports which are directly based on one’s own interests, preferences, strengths and capacities.
I AM’s philosophy is that even though a person might not speak, they must be heard. Now, with Individual Support Plans based upon person-centered assessment data, services can be provided that not only address an individual’s health and safety requirements, but their desires.
I AM is an easy-to-use iPad app that provides the most meaningful IDD assessment available. With I AM, Care Coordination Organizations can ensure individuals receive the services they need by empowering providers to meet today’s higher standards of support and providing more person-centered services.
